Instructions and information needed for submitting an HR Service Request through Ivanti

Below are instructions and information that is needed for submitting a Service Request form in Ivanti for UW Parkside HR. 

Request for Medical Accomodations (Confidential)

*Note - This form is confidential and will only be seen by the HR manager

Information required:

  • Employee Email
  • Employee Phone Number
  • Description of the qualifying disability
  • Is this condition permanent?
  • Discription of how the disability impairs your ability to perform assigned job duties
  • Describe the reasonable accommodation you are requesting
  • Medical Certification Form - ADA Accomodation Medical Certification

How to Submit the form:

  • Go to Ivanti - Ivanti Login
  • If you have multiple roles, select Self Service
  • Click Service Catalog
  • Search "PKS" in the search bar
  • Click "PKS - [Confidential] Request for Medical Accommodations"
  • Fill out the required information

What's Next:

  • You will recieve an email after your request has been submitted
  • If HR requires addtional information, you will receive an email requesting the additional forms or information
  • The request will be routed to HR for approval
  • You will recieve an email letting you know if the request is approved or denied

 

Person of Interest/Volunteer/Contingent Worker Form

Information required:

  • Requested Action (What type of Account are you requesting)
  • Department contact email
  • Department contact name
  • Department Code
  • Start Date
  • End date (If no end date is entered appointment will end in 1 year)
  • Will this person have access to minors where they will be actively working with and/or supervising minors (17 and younger) in a youth event? (Yes/No)
  • Does this volunteer need access to UWP programs/email?(Yes/No)
  • POI/Volunteer First Name
  • POI/Volunteer Last Name
  • POI/Volunteer Personal Email

How to Submit the form:

  • Go to Ivanti - Ivanti Login
  • If you have multiple roles, select Self Service
  • Click Service Catalog
  • Search "PKS" in the search bar
  • Click "PKS - Person of Interest/Volunteer/Contingent Worker Form"
  • Fill out the required information

    What's Next:

    • You will recieve an email after your request has been submitted
    • An email will go to the POI requesting information
    • The request will be routed to HR for approval
    • You will recieve an email letting you know if the request is approved or denied
    • The appointment will have to be entered into SOLAR/SIS
      • Once appointment is entered there is an automated workflow that takes place within 48 hours to populate and activate the account

     

    Harassment Complaint Form (Confidential)

    *Note - This form is confidential and will only be seen by the HR manager

    Information required:

    • Employee Name
    • Employee Email
    • Employee Phone Number
    • Employee Address
    • Work Unit
    • Supervisor Email
    • How you are related to the university community (list provided in Ivanti)
    • Is this in regard to employees, educational programs, services or activities
    • The protected status or activity you believe prompted the alleged discrimination, harassment, or retliation (List provided in Ivanti)
    • Short clear statement of your complaint
      • You may create your own and attach it with this form if you need more space. The following information should be included in your statement:

        Your contact information, including full name, local address, telephone number, and email address;

        The name and job title of the offending person and the relationship to complainant (e.g., supervisor, dean, co-worker, etc.). If there is more than one person named in the complaint, please submit a separate statement for each person. If you are filing a complaint on someone else’s behalf, please also provide the name and contact information for that person.

        The protected status of the complainant that led to the alleged discrimination, harassment, or retaliation;

        For each protected status, a description of why you believe the actions of the accused toward you were based on that protected status;

        The dates, time period, and the location of each of the actions or incidents believed to be discriminatory;

        The effect that the actions or incidents have had on the complainant’s work, instructional, or study environment, or the complainant’s ability to take part in university programs or activities;

        The names and contact information for individuals who may have information regarding the allegations;

        The names of any individuals who are similarly situated and who might have experienced similar treatment;

        Other relevant information supporting the allegations (do not refer to supporting materials in lieu of describing them in your statement); and sign and date the complaint and deliver it to the Office of Human Resources, 900 Wood Road, Kenosha, WI 53140. 262-595-2204

    • If you have previously reported this concern to a designated university official
    • What actions, if any, have been taken so far by university officials regarding this concern?
    • Have you discussed this concern with any other member of the university community other than a designated official? (i.e. co-workers, fellow students, attorneys, EAP, etc.)
    • Have you filed a complaint or grievance related to this situation through other internal processes (i.e. Faculty, Academic Staff, or Classified grievance processes) or with an external agency such as

      Information required:

      • Was the crime a felony or misdemeanor (including DUI/OWI)
      • Conviction
      • Conviction Type
      • Conviction Date
      • County
      • State
      • Description of charges and conviction – provide details of all offenses including nature, circumstances, and dates

      • Type of leave to be used (Can select multiple - Sick Leave, Vacation, Personal Holiday, Unpaid Leave)
      • Amount of hours for each type of leave selected
      • Start Date
      • End date

      Non-medical Leave Request

      *Note -  Please note that approval of an unpaid leave of absence is not guaranteed. We highly recommend that you meet with a Benefits Specialist prior to requesting a non-paid leave of absence.
